Checking Out and Discharge
Important Patient Information
In preparation for discharge, you will receive instructions about follow-up care, such as diet, medications, level of activity and follow-up appointments with your physician. A nurse or physician will review these instructions with you. Please ask them about any concerns you may have.
If you have home health care or home medical equipment needs, the Social Work Services department (956-523-2510) will assist you during the discharge process.
