Checking Out and Discharge In preparation for discharge, the staff will work with you to prepare for your return home. You will receive instructions about follow-up care, such as diet, medications, level of activity, and follow-up appointments with your physician. A nurse or physician will review these instructions with you. Please be sure to ask them about any concerns you may have. If you have any special needs such as home health care or home medical equipment, the Social Work Services department will assist you with these needs during the discharge process.
